Narrowing of the eye opening is not always due to drooping eyelid

The eye opening may appear narrower or the eyelid may seem droopy, but this appearance does not always mean there is a drooping eyelid.

The shape of the eye opening is influenced by several factors, including the level of the eyelid, the position of the eyebrow, the amount of skin above the eyelid, the position and movement of the eye, in addition to the function of the muscles surrounding the eyelid and eye.

Therefore, eyelid drooping may be the cause, or another factor may contribute, or several factors may combine to give the same appearance.

When multiple factors combine

Sometimes, there are multiple factors that lead to narrowing of the eye . For example, as we age, several changes can occur in the area around the eye at the same time. There may be drooping of the eyelid, along with sagging of the skin above it, and a gradual decline in the level of the eyebrow. When these changes combine, the eyelid may appear more droopy, and the eye opening may become smaller than it would be due to eyelid drooping alone.

This is something we see in the clinic frequently. For example:

A patient may come in with a noticeable drooping of the eyelid, and the cause is indeed age-related eyelid ptosis.

In other cases, there may be actual drooping of the eyelid, in addition to sagging of the skin above the eyelid ( Dermatochalasis ) , with both contributing to the narrowing of the eye opening.

There may be actual eyelid drooping with a decline in eyebrow level ( Brow ptosis) which increases the heaviness of the area above the eye and makes the appearance of drooping more noticeable.

In some cases, eyelid drooping may not be the primary problem, but rather a change in the position of the eye, such as a decline in the eye's position in certain types of strabismus, is the main factor contributing to the narrowing of the eye opening.

Why do we care about identifying the cause?

Because similar appearances do not mean that the treatment is the same.

Treatment for eyelid drooping differs from treatment for skin sagging, and dealing with eyebrow decline differs from both. Also, the presence of a problem related to the position of the eye or the function of the surrounding muscles can greatly change the diagnosis and treatment plan.
